How to fill out an application for a visa to Poland?

If you have come to the stage of preparing documents for obtaining a visa to Poland, you will be faced with the need to fill out a visa application form. Whether you are submitting documents to receive or on the basis of the Pole's Card, whether you want to receive a Polish Schengen for the purpose of tourism, you will not be able to bypass the stage of filling out the questionnaire.

What should not be forgotten? Mistakes in the visa application form can lead to refusal, so be extremely careful when filling it out.

How to fill out an application for a visa to Poland - a detailed guide

As for obtaining a visa to any of the countries that have signed the Schengen Agreement, the visa application form for Poland should be filled out in electronic form on the official website of the Polish Ministry of Foreign Affairs.

To do this, follow the link -https://secure.e-konsulat.gov.pl . Below is a sample of filling out a questionnaire for a person who plans to travel to Poland to work in a construction company at the invitation of an employer (multiple entry visa 180/360, category D - national).

When filling out the questionnaire, do not forget about the time. The system provides 30 minutes for filling out one questionnaire. If you do not meet this time, the whole procedure will have to start from the very beginning.

Stage 1. We select the country (in our case - Ukraine) and in the next window - we select the consulate we need (Vinnitsa, Kyiv, Lutsk, Lviv, Odessa or Kharkov). We remind you that you can submit documents only at the Consulate at the place of registration or place of official employment.

On the next page, you make a choice of the category of visa (national or Schengen), enter the "captcha" - the verification code in the picture and press the button " Further«.

Let's stop a little at this stage and figure out which category of visa to choose and which link to follow:

  • if you are applying for a national visa for the purpose of going to work, follow the very first link " National visa - register the form«.
  • for tourists - link number 3 "Schengen visa - register the form»
  • for owners and those who submit documents at the invitation of relatives, there is no need to go through the standard procedure. In this case, you follow the second link "Fill out the form for a National visa", fill it out and print and submit documents on any day convenient for you. (This does not apply to applicants from the Vinnytsia District - they follow the standard procedure.)

Stage 2. Date selection. You have two options for the development of events. Option one is unfavorable if the following message appears in the window:

If there are available dates for registration, we select the appropriate day for visiting the consulate.

Stage 3. We proceed to the direct filling out of the questionnaire.

The first screen - personal data(fill in the same way as indicated in your passport):

  • Surname (in Latin letters)
  • Generic (maiden) surname - if the surname has not changed, duplicate from the first column
  • Date of birth in year-month-day format
  • Place of birth (name of locality)
  • Country of birth (select from the drop-down list)
  • Select the country of which you are a citizen from the drop-down list
  • Select from the drop-down list the country of which you were a citizen at birth
  • mark gender
  • celebrate marital status
  • TIN (individual identification number) - 10 characters

Screen number 2. Travel document details

  • If you are traveling on a standard passport, check the first item "regular item"
  • Series and number of the passport
  • date of issue in the format (year-month-day)
  • Passport expiration date (year-month-day)
  • The authority that issued the passport (in digital format)

Screen 3: Home Address

  • field No. 10 is filled in exclusively for minors. In our case, check the box "Does not apply"
  • We fill in the Cyrillic address of your residence (registration), e-mail and contact phone number (field No. 17)
  • field No. 18 - If your country of residence differs from the country of citizenship, fill out this section, otherwise mark "NO"

Step 4. Employer details

  • in field No. 19, select from the drop-down list information about your specialization (profession) at the time of filling out the questionnaire
  • field number 20. Information and contact details of the future employer in Poland (for those traveling to work) or educational institution (for those traveling to study). Schengen Visa applicants or Pole's Card holders do not complete this section.

Stage 5. Information about the trip.

In field number 21, mark the purpose of the trip. For applicants traveling to work in Poland, check the box “Other” and enter the word “PRACA” in the field. For owners of the Pole's Card, enter in the field "KARTA POLAKA".

  • Field No. 22. The country of destination is Poland and the country of first entry is also Poland.
  • field 24 - how many times do you plan to cross the border (please note - multiple times), for a tourist trip - once;
  • field 25 - period of stay in Poland. To go to work by invitation - 180 days, for Pole's Card holders - 360 days, for tourists - depending on the duration of the trip
  • field 29-30 - planned dates of arrival and departure from Poland;
  • field 26 - have you received Schengen visas before (if yes - indicate the validity period, if not - mark the item "NO");

  • field 27 - were you fingerprinted for biometrics earlier when opening a Schengen visa?

Field 31-32— Data of the receiving party:

  • for those traveling to work - information about the employer and his contact numbers (with country code - 48)
  • for tourists and holders of the Pole's Card (information about the hotel where you plan to stay - you can enter the contacts of any hotel from the Booking.com service)

Field No. 33 - Who bears the costs associated with the trip and information about insurance

Often, the field is marked that the applicant himself bears the expenses and the items “Cash” and “credit cards” are ticked. If you are visiting by invitation, you can specify the details of the host.

According to insurance - it must be issued for the entire period of the trip. However, there is a nuance here:

  • If you are going to work by invitation and you have a six-month visa, insurance must be issued for 180 days with a corridor of 360 days;
  • if you have a work permit () - insurance can be issued for 10 days with a corridor of 30 days. After all, it is understood that subsequent insurance is the concern of the employer.

Item No. 34 is marked with a tick in the field "DOES NOT CONCERN". You should also check the next 3 boxes, where you confirm that you are warned:

  • the consular fee is not refundable even in case of refusal;
  • about insurance;
  • about the reliability of the specified data.

After you click the "SAVE" button, it will be assigned a unique number, an individual barcode and the date / time of receipt at the Polish Consulate. Print the questionnaire - it should contain 3 pages - the third page will contain a barcode (bar code). Do not forget this page - without it, documents will not be accepted for work.
